Yes. Patients who wish to continue their care with Dr. Upitis may schedule appointments at her new Oak Ridges practice. It is requested that you have your Family Physician send a new referral to provide continuity of care. Please request that all pathology/laboratory reports be sent.

If you would like your records transferred, please contact our office. You may be asked to complete a medical records release form so that your information can be securely sent to the new clinic. There is a small fee associated with this service, unless your primary care giver sends a new report with your medical record.
