Can I continue my care with Dr. Upitis?

Yes. Patients who wish to continue their care with Dr. Upitis may schedule appointments at her new Oak Ridges practice. It is requested that you have your Family Physician send a new referral to provide continuity of care. Please request all pathology/laboratory reports to be sent

How do I transfer my medical records to the new clinic?

If you would like your records transferred, please contact our office. You may be asked to complete a medical records release form so that your information can be securely sent to the new clinic. There is a small fee associated with this service, unless your primary caregiver sends a new report with your medical record.

What if I prefer to continue care at Toronto Dermatology Centre?

We welcome you to stay at Toronto Dermatology Clinic. Our team will be happy to assist you in continuing your dermatology care with another physician at Toronto Dermatology Centre.

What happens to my upcoming appointment with Dr. Upitis?

Our office will contact patients with upcoming appointments to discuss available options and help coordinate care.
